Northland Shepherd’s Center (NSC) is seeking a compassionate and resourceful Aging Resource Coordinator to support older adults in Clay and Platte counties. This vital role serves as a navigator and advocate for clients, guiding them through available programs, resources, and services to address the challenges of aging.
You’ll be responsible for assessing client needs, managing a caseload, coordinating support, and collaborating with internal teams and external partners. The ideal candidate brings strong communication skills, problem-solving abilities, a heart for service, and a desire to lead with kindness. Supervisory experience, data tracking, and a knowledge of aging-related resources are ideal in this position.
Key Responsibilities
- Manage a caseload of clients, tracking progress and navigating complex situations with sensitivity and discretion
- Conduct individualized needs assessments and benefits reviews—including in-home visits—to develop personalized plans
- Enroll clients in NSC and partner programs, assist with documentation, and ensure timely follow-up and communication
- Maintain accurate, confidential records of client interactions, services provided, and ongoing needs.
- Stay informed about local resources, services, and benefits, and serve as a knowledge hub for the community. Provide outreach to identify and engage potential clients, and represent NSC at community events and forums
- Collaborate with internal teams, volunteers, and external providers to deliver wraparound support and improve outcomes. Communicate regularly with staff about client progress, needs, and service coordination to ensure a seamless support experience
- Develop and manage program budgets, and prepare data-driven reports for funders and leadership.
- Contribute content for newsletters and social media to share resources, client stories, and program updates
- Train and supervise Resource Specialists and volunteers assisting with client services
- Perform other duties as needed to support the mission and operations of NSC
Ideal Qualifications
- Knowledge of aging-related services, public benefits, community resources, and challenges affecting older adults in Clay and Platte Counties
- Experience working with older adults or vulnerable populations
- Degree in social work, gerontology, or related field; relevant experience or certification considered in lieu of a degree
- Experience with care or case management, client intake, care coordination, problem-solving and advocacy
- Compassionate, positive, client-centered approach with a commitment to service and a strong sense of ethics and accountability
- Strong written and verbal communication skills
- Organized and detail-oriented, with the ability to manage multiple tasks and priorities
- Proficient with Microsoft Office Suite; willingness to learn client management software
- Must have a valid Missouri driver’s license, reliable transportation, and auto insurance
Job Details
Location: Gladstone, Missouri
Schedule: Monday–Friday, 35–40 hours/week, on-site with 8:30am start
Reports to: Executive Director
Pay: $23–$25/hour | Benefits include PTO and paid holidays
