Reentry Case Manager

Journey to New Life is hiring a Reentry Program Case Manager. The ideal candidate would be someone who has strong advocacy skills and is passionate about helping clients overcome the barriers they face after incarceration. People with lived experience of incarceration are strongly encouraged to apply.

Our mission is to create pathways for recently incarcerated people to overcome reentry barriers and live fulfilling lives.


Agency description:
Journey to New Life is a peer-led reentry program that guides individuals through the transition from incarceration to community integration by providing wrap-around services, including emergency assistance, housing assistance, case management, employment services, and peer support. Beyond direct assistance to individuals, we advocate for our clients in systems where they face discrimination and support policy reform in areas that directly impact their lives.

                                                         
Responsibilities:

  • Goal Oriented Case Management: Case Managers use a strengths-based, solutions-focused approach and support clients in their identified goals. They connect clients to appropriate community resources, such as financial assistance, recovery services, mental health treatment, housing, employment, healthcare, and to address other needs as identified.  They collaborate with community partners to ensure that interventions and community support services are available and easily accessible. They also provide crisis intervention services as needed.
  • Advocacy Services: Case Managers provide support and advocacy services for clients concerning mental and physical health services, educational needs, legal and housing assistance, employment search and support, and other areas as identified by clients.
  • Teamwork: Case Managers collaborate as a member of an interdisciplinary team within the agency. They attend team meetings, help form program policies and procedures, and maintain positive relationships with their coworkers. Case Managers also support the agency’s community outreach and relationship building, as well as the facilitation of peer support groups, as needed.
  • Group Facilitation: Case Managers facilitate educational groups for both their case management clients and clients receiving services in other JTNL programs. Education is a big piece of the transformation and stabilization process and essential to upholding the Mission and Vision of JTNL.
  • Professionalism: Case Managers communicate well with coworkers and clients. They respond to their voicemails, texts, emails, and messages in Teams each day. They represent the agency well in the community. Case Managers set healthy boundaries with clients and always protect their confidentiality.
  • Documentation: Case Managers maintain accurate and timely documentation regarding communication and activities with clients and services received, including completing required reports, in accordance with policies and procedures and specific funding sources.  
Qualifications: 
  • Commitment to the mission of JTNL
  • Must adhere to agency’s non-discrimination policy and be able to effectively interact with co-workers and residents with diverse backgrounds, including but not limited to religious views, political affiliation, cultural background, lifestyle, sexual orientation, gender identification
  • Personal traits: empathetic, kind, open-minded and willing to learn new skills
Education and Training
  • Bachelor’s Degree in social work or related field (2 years of relevant work experience may be substituted for a degree)
  • Must complete Peer Support Certification or Missouri Recovery Support Specialist within the first 6 months of employment.
  • Must complete certification in CPR within the first 6 months
Experience
  • Experience with individuals who have had involvement in the legal system
  • Awareness of the importance of recovery services
  • If in recovery, able to demonstrate at least two years of continuous sobriety
  • Preference given to persons formerly incarcerated       
Skills
  • Ability to work well with people from all backgrounds
  • Familiarity with the legal system and how to support people in re-entry
  • Ability to work independently and as a member of a team
  • Excellent verbal, interpersonal and written communication skills
  • Demonstrated ability to utilize computers and proficiency in Microsoft Office
  • Capacity to learn relevant client management programs including CaseWorthy
  • An understanding of substance abuse problems and treatments
  • Ability to facilitate access to local community-based resources
  • Ability to lift 40lb
